Beneficiary Application Form

Children’s Education Funds Inc. (CEFI) welcomes you to our online Beneficiary Application Form process for you to apply for the 2017 Education Assistance Payment (EAP). To apply for an EAP you need to complete and submit the Beneficiary Application Form and you must request the Registrar’s Office at your post-secondary school to complete an Enrolment Confirmation Form. Both Forms must be provided to CEFI by August 1, 2017.
 
Please complete all required fields of the Beneficiary Application Form. Once completed, please click "Submit" to send the Beneficiary Application Form to CEFI. Kindly ensure that the completed form is submitted to CEFI by August 1, 2017.
 
Also, please be sure to forward the instructions that were previously mailed to you to the Registrar’s Office at your post-secondary Institution as they provide important information regarding how to complete and submit your Enrolment Confirmation Form.
